Complete the provider registration form with your NPI, state license, and practice information. NPI verification (NPPES), OIG LEIE exclusion check, SAM.gov debarment check, and PECOS enrollment lookup all run automatically at submission.
Credentialing Review
Our credentialing team reviews your application within 1-2 business days. Exclusion flag matches route to manual review before a final decision. Clean applications receive expedited review.
Trial Begins
The moment your application is approved, your 90-day trial starts automatically. No billing, no contracts, no commitments. You receive a confirmation with portal credentials.
After Your Trial

After your 90-day trial, select a paid plan to maintain uninterrupted access. Choose from Solo Provider, Small Practice (2–10 providers), or FQHC / Multi-Site based on your organization size. Patients enrolled during your trial are never affected — their records, consent history, and blockchain audit trail remain intact under your paid subscription. Pricing is currently being finalized.
