1. Acceptance of Terms
By accessing or using the MyRxWallet platform — including the patient portal at patients.myrxwallet.io, the provider portal at providers.myrxwallet.io, the public website at myrxwallet.io, or any associated mobile application or API — you agree to be bound by these Terms and Conditions (“Terms”). If you do not agree, do not use the Platform. These Terms apply to all users, including patients, veterans, healthcare providers, and any other authorized users.
2. Platform Description and Eligibility
MyRxWallet is a HIPAA-compliant, blockchain-anchored health and benefits management platform that allows enrolled members to aggregate health records, manage pharmacy data, track VA benefits and claims, access veteran-specific features, and share records with authorized healthcare providers. The Platform is designed for adults 18 years of age and older. Use by or on behalf of minors requires documented parental or legal guardian consent.
MyRxWallet is not a medical device, does not provide medical advice, and is not a licensed medical provider. Nothing on the Platform constitutes a diagnosis, treatment recommendation, or substitute for professional medical judgment.
3. Account Registration and Security
You must provide accurate, complete, and current information during registration. You are responsible for maintaining the confidentiality of your account credentials, including your wallet seed phrase if applicable. You agree to notify MyRxWallet immediately at support@myrxwallet.io of any unauthorized access to your account. MyRxWallet is not liable for losses resulting from unauthorized use of your credentials.
Patient identity is anchored to a non-transferable digital identity credential (ERC-5192 soulbound, Chain 8472) issued upon enrollment. This credential cannot be transferred to another wallet. Loss of wallet credentials may result in inability to recover your account without identity re-verification.
4. Health Data and HIPAA Authorization
By enrolling on the Platform, you authorize MyRxWallet to act as your HIPAA Business Associate and, where applicable, as a HIPAA-covered entity in connection with your health data. You acknowledge and consent to the collection, storage, and processing of your Protected Health Information (PHI) as described in our Privacy Policy and HIPAA Notice of Privacy Practices.
You may authorize MyRxWallet to retrieve your health records from third-party sources including CMS (Medicare via Blue Button 2.0), commercial insurers (via FHIR R4 payer APIs), and the U.S. Department of Veterans Affairs (via VA Lighthouse APIs). Each authorization requires your explicit consent and may be revoked at any time from the Consents tab in your portal. Revocation is effective immediately and logged on the blockchain.
5. VA Lighthouse API Data and Veteran Features
Veterans who connect their VA data authorize MyRxWallet to access their records via the VA Lighthouse Authorization Code Grant (ACG) OAuth 2.0 consent flow. By granting this authorization, you agree to the following terms:
- Data scope: MyRxWallet may access your VA FHIR R4 health records (diagnoses, medications, lab results, immunizations, allergies, clinical notes), military service history (service periods, branch of service, discharge character), VA disability rating, VA healthcare enrollment status, community care eligibility status, and VA benefits claims status, as permitted by your authorization scope
- Purpose limitation: VA data is used solely to populate your sovereign health wallet, power the PACT Act Automatic Eligibility Screening engine, and surface VA benefits relevant to your service profile. VA data is never used for advertising, never sold to third parties, and never shared without your explicit written consent
- PACT Act Screening: The PACT Act Automatic Eligibility Screening feature cross-references your confirmed VA service history against the statutory criteria of the Sergeant First Class Heath Robinson PACT Act of 2022 (Public Law 117-168). Results are provided for informational purposes only. MyRxWallet is not a VA-accredited claims agent and does not represent veterans before the Department of Veterans Affairs. Eligibility determinations by the Platform are not binding on the VA and do not constitute an official VA eligibility determination
- Claims display: VA benefits claims data is displayed for informational purposes only. MyRxWallet is not a Veterans Service Organization (VSO) and does not provide claims representation, legal advice, or appeals assistance. All claims decisions remain with the VA
- Ethics Principles: MyRxWallet has attested to the VA Ethics Principles for Access to and Use of Veteran Data (VA Lighthouse Case #00015333) and will not use veteran data for any purpose not expressly described in this Agreement and our VA Lighthouse application
- Revocation: You may revoke VA data access at any time from your patient portal or via va.gov. Revocation is effective immediately. MyRxWallet will cease accessing new VA data and will process deletion requests per our Privacy Policy within 30 days
6. Blockchain Identity and On-Chain Records
Your MyRxWallet identity is anchored to a non-transferable digital identity credential (ERC-5192 soulbound). No personally identifiable information or PHI is stored on-chain; only cryptographic hashes are recorded. On-chain consent records are permanent and immutable. Off-chain health data is stored in encrypted databases on hardened infrastructure and may be deleted upon request per our Privacy Policy.
7. Provider Features and Clinical Responsibilities
Healthcare providers accessing patient data through the provider portal may do so only with valid, unexpired patient consent on file. Providers are solely responsible for all clinical decisions made on the basis of information accessed through the Platform. MyRxWallet does not practice medicine and accepts no liability for clinical decisions. Providers must maintain all applicable professional licenses, certifications, and malpractice coverage independent of Platform use.
8. Prohibited Uses
You agree not to use the Platform to:
- Access, copy, or modify another user’s health records without authorization
- Circumvent, disable, or reverse-engineer any security or authentication feature
- Upload or transmit malicious code, viruses, or disruptive programs
- Use VA data for any purpose other than those described in Section 5 and our VA Lighthouse application
- Sell, license, or transfer veteran data obtained through VA Lighthouse APIs to any third party
- Use the Platform for any unlawful purpose or in violation of HIPAA, HITECH, or applicable federal and state law
- Impersonate another person, provider, or entity
- Scrape, harvest, or bulk-download health data via automated means
- Use PACT Act eligibility results for any commercial purpose not authorized in writing by MyRxWallet
9. Intellectual Property
The MyRxWallet platform, website, software, and all associated content (excluding your personal health data, which you own) are the intellectual property of MyRxWallet North America Corporation. You are granted a limited, non-exclusive, non-transferable license to use the Platform for its intended purpose. Your personal health data remains yours — MyRxWallet claims no ownership over your PHI or health records.
10. Disclaimers
THE PLATFORM IS PROVIDED “AS IS” AND “AS AVAILABLE.” MYRXWALLET MAKES NO WARRANTY, EXPRESS OR IMPLIED, THAT THE PLATFORM WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE FROM SECURITY VULNERABILITIES. HEALTH INFORMATION DISPLAYED ON THE PLATFORM IS DERIVED FROM THIRD-PARTY SOURCES (VA LIGHTHOUSE, CMS, COMMERCIAL PAYERS) AND MAY NOT REFLECT THE MOST CURRENT CLINICAL DATA. PACT ACT ELIGIBILITY DETERMINATIONS PROVIDED BY THE PLATFORM ARE FOR INFORMATIONAL PURPOSES ONLY AND DO NOT CONSTITUTE OFFICIAL VA DETERMINATIONS. ALWAYS CONSULT A LICENSED HEALTHCARE PROVIDER FOR MEDICAL DECISIONS AND A VA-ACCREDITED REPRESENTATIVE FOR CLAIMS ASSISTANCE.
11. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, MYRXWALLET NORTH AMERICA CORPORATION, ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF OR INABILITY TO USE THE PLATFORM, INCLUDING ANY RELIANCE ON HEALTH DATA, VA DATA, OR PACT ACT ELIGIBILITY DETERMINATIONS PROVIDED THROUGH THE PLATFORM. MYRXWALLET’S TOTAL LIABILITY TO YOU SHALL NOT EXCEED THE GREATER OF (A) THE AMOUNT YOU PAID FOR PLATFORM SERVICES IN THE TWELVE MONTHS PRECEDING THE CLAIM OR (B) ONE HUNDRED DOLLARS ($100). NOTHING IN THESE TERMS LIMITS LIABILITY FOR GROSS NEGLIGENCE, WILLFUL MISCONDUCT, OR AS OTHERWISE PROHIBITED BY APPLICABLE LAW.
12. Indemnification
You agree to indemnify and hold harmless MyRxWallet North America Corporation and its officers, directors, employees, and agents from any claims, damages, losses, and expenses (including reasonable legal fees) arising from: (a) your use of the Platform in violation of these Terms; (b) your submission of false, inaccurate, or unauthorized information; (c) your misuse of VA data; or (d) your violation of any applicable law or regulation.
13. Termination
MyRxWallet reserves the right to suspend or terminate your account if you materially violate these Terms, including misuse of VA data, HIPAA violations, or unauthorized access to other users’ records. You may close your account at any time by contacting support@myrxwallet.io. Upon termination, VA and all other third-party data authorizations are revoked and your PHI is deleted per our Privacy Policy retention schedule, unless retention is required by applicable law.
14. Governing Law and Dispute Resolution
These Terms are governed by the laws of the State of Nevada, without regard to conflict of law principles. Any dispute arising from these Terms or your use of the Platform shall be resolved by binding arbitration under the American Arbitration Association Commercial Arbitration Rules, conducted in Clark County, Nevada. You waive any right to participate in a class action lawsuit or class-wide arbitration. Nothing in this section prevents either party from seeking injunctive relief in a court of competent jurisdiction to prevent irreparable harm.

A-2. Program Description
Transactional SMS messages from MyRxWallet Health Platform including:
- One-time passcodes for account verification during patient and provider login;
- Confirmation of support tickets submitted via the contact form at myrxwallet.io/contact.html;
- Callback scheduling confirmations for user-initiated callback requests;
- Related account notifications directly responsive to user actions on the platform.
All messages are transactional in nature. MyRxWallet does not send marketing or promotional SMS messages.
A-3. Message Frequency
Message frequency varies based on user activity. Typical volume is 1–5 messages per event (account signup, support ticket submission, callback request). Users receive messages only in direct response to actions they initiate on the platform. No recurring or unsolicited messages are sent.
A-4. Message and Data Rates
Message and data rates may apply. Your wireless carrier may charge for incoming and outgoing text messages depending on your plan. MyRxWallet is not responsible for carrier charges.
A-5. How to Opt Out (STOP)
To stop receiving SMS messages at any time, reply STOP to any message. You may also reply UNSUBSCRIBE, CANCEL, END, or QUIT. You will receive a final confirmation message, after which no further SMS will be sent from MyRxWallet to that number.

A-8. Message Delivery
Message delivery is not guaranteed and depends on your mobile carrier. MyRxWallet is not responsible for delays or failures caused by carrier network conditions, geographic coverage limitations, or device incompatibilities. Delivery to T-Mobile users is subject to T-Mobile’s terms; T-Mobile is not liable for delayed or undelivered messages.
A-9. Eligibility
Users must be 18 years or older, or have the consent of a legal guardian, to provide a phone number and receive SMS messages from MyRxWallet. By submitting your phone number, you represent that you meet this eligibility requirement or that you have obtained appropriate parental or guardian consent.
A-10. Privacy
Your phone number and contact information are processed in accordance with the MyRxWallet Privacy Policy. We do not share, sell, rent, or lease your phone number to third parties for marketing purposes.
A-11. Changes to These Terms
MyRxWallet reserves the right to modify these Terms and Conditions at any time. Material changes will be communicated via the website at myrxwallet.io and, where required by applicable law, by direct notice to affected users. Continued use of the SMS program following notice of material changes constitutes acceptance of the updated terms.
A-12. Governing Law
These Terms are governed by the laws of the State of Wyoming, United States of America. MyRxWallet North America Corporation is a Wyoming-domiciled corporation. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts of the State of Wyoming.
